Can someone send me the template too? I'm facing the same problem and need to get approve soon. Do i have to make a trip down to HDB Hub and speak to the officer personally?

Hi in the end i didnt get any template. Just wrote a brief letter stating I need to defer the ROM cert. Remember to bring ur receipt for hotel showing proof when ur wedding will be to the 1st appt can liao.
